Even worse, the helicopter fell on a residential area killing civilians and children in a kindergarten. A number of theories was considered by investigators, including sabotage, technical failure and human mistake.
Preliminary results of flight recorder decoding just completed indicate the accident was what in air safety is called controlled flight into terrain — when an operational aircraft crashes into the ground while being controled by a conscious pilot, usually as result of pilot's loss of orientation or mistake. In this case, the pilot was not given detailed information on weather conditions, dense fog, and flew in low altitude. When he saw contours of residential houses in front of him, he started a maneuver to bypass them, but lost control and crashed.
Ukraine's investigation charged five people from various offices involved in the flight logistics with negligence and violation of air safety rules, which were numerous in this case.
The helicopter was an emergency one, not certified for long distance flights. First pilot did not receive detailed briefing on difficult weather conditions and didn't plan the route accordingly, since he wasn't trained for piloting in such conditions. Plenty of people could have rescheduled that flight for later hour, but didn't.

In Poland the crash was almost entirely blamed on people who died (initially) and then on a convoluted Russian conspiracy (when PiS won elections), creating a whole sectarian "Smolensk cult" (and the wreck still remains in Russia, who claims they still "continue investigation").
