The problem with postmodernism is that it allows people to take extremely radical positions without creating positive change.

But just because you’re critiquing a power structure doesn’t mean you’re challenging or negating it. On the contrary, sometimes by critiquing a power structure, you’re finding more effective ways to exploit it.

For example, BuzzFeed co-founder Jonah Peretti did a great job of critiquing the power structure of identity and formation. Through a Marxist lens, he uncovered how the Internet can exploit this power structure.

So what did he do? He formed BuzzFeed, and profited handsomely. BuzzFeed became famous for its online activism—exploiting people’s sense of identity and formation. Nothing changed but Jonah Peretti got rich.

{Acronym alert - glossary at bottom.} I see more patients getting bumped from ICS-only monotherapy (and/or SABA-only rescue med) to combo ICS-LABA inhalers. This is spurred by GSK discontinuing brand-name Flovent, and GINA guidelines promoting AIR and MART (aka SMART) strategy, which utilizes a combo inhaler containing formoterol, like Symbicort, Breyna, or Dulera. This is a big change for many people, who're used to using an inhaled for control and prevention, and inhaled for rapid relief. Now they can use one inhaler for both -- which is convenient, and may bring better control.

Now I'm also seeing PCPs, pediatricians, and pulmonologists prescribing other combo ICS-LABA inhalers like Advair with a SMART asthma action plan -- even though that's not approved under GINA or NAEPP guidelines. In fact they explicitly warn against this: "Combination ICS with non-formoterol LABA cannot be used as-needed for symptom relief." (GINA main report, p. 69). LABAs like salmeterol (more lipophilic) take much longer to kick in (30+ minutes) to be intended for rapid relief, and have not been studied for rescue use. But they're being prescribed that way anyway, presumably due to availability, insurance coverage, and clinician confusion during a time of flux. Leading to a lot of follow-up conversations. :blobcatshrug:​

Glossary (in no order):

  • ICS == inhaled corticosteroid (Flovent, QVAR). Steroids reduce airway inflammation slowly over time.
  • SABA == short-acting beta-agonist: a bronchodilator (canonically albuterol) that quickly relieves airway constriction, for a few hours.
  • LABA == long-acting beta-agonist (ex: salmeterol, formoterol), relieves airway symptoms for more than a few hours.
  • Rescue med, aka "reliever" == a rapid-acting drug to relieve acute symptoms (e.g. albuterol, epinephrine).
  • Maintenance therapy == drug you take daily to prevent acute symptoms.
  • Monotherapy == managing a disease with just one drug.
  • AIR == Anti-Inflammatory Reliever: combo inhaler with a low-dose steroid + a rapid-acting bronchodilator
  • MART == Maintenance And Reliever Therapy: treatment with an inhaler that contains both a steroid and a rapid-onset, long-acting beta-agonist (formoterol). Also called SMART (S == "Single-inhaler").
  • GINA == Global Initiative on Asthma

CDC is working to expand the scope of technical backgrounds of participants on the HICPAC Isolation Guideline Workgroup and eventually among the committee members through established processes in accordance with the Federal Advisory Committee Act (FACA) regulations and guidance. The expanded workgroup and the HICPAC with the newly appointed members will review and discuss these additional considerations and guideline at the next HICPAC meeting, which is open to the public.

The Fediverse increasingly has worse disinformation than other spaces. The problem is, the users don't read the news very much, and many have a culture that rejects reading and discussing news. So the pool of people who talk about the news is too small for fact checks. Media literacy here is so low that people go viral with massacre denialism because they read something in Middle East Eye and don't know it's Qatar's Global Times, a propaganda outfit used for things Al-Jazeera won't touch.

@Alon I'm not active on any other social network these days. How could one tell if the problem was worse in a feed here than it would be in on FB, X, Threads, etc, driven by opaque algorithms. I suppose on the fediverse, you could direct your concerns to "super boosters" (e.g. Lisa Melton). But anyway, the people I follow seem to have a pretty high level of media literacy, at least compared to the people (and some bots) who comment on articles in commercial media outlets.

Every Kryptonite U-lock I've owned develops problems turning the key in the lock after I've owned it for a few years. I got replacement keys earlier this year, which started demonstrating the same problem about 2 months ago. I recently realized that I can get the key to turn in my series 2 lock much more reliably if I orient the whole lock so the crossbar is down (keyhole opening facing the ground), and shake it downward a little as I turn. Just mentioning this here in case it proves useful to anyone else. #BikeTooter #cycling

I know exercise while having symptoms is associated with making long covid worse - but assuming I fully recover in a week or so, how long should I give it before returning to the pretty intense kettlebell workouts I was doing 3x/week?

@misc Kettlebell seems like it would be particularly rough, likely to provoke positional BP and heart rate changes which are frequently hard on people with #longcovid and #MECFS ("orthostatic intolerance"), even w/o weights.

There are many researchers arguing about this topic (and whether it's even safe to study exercise in LC patients), so it's very hard to give good recommendations aside from "go slow" and consult with your PCP.

I've read recommendations to start off with rowing or recumbent cycling, at low duration and low intensity for several weeks, transitioning to upright exercise at 9-12 weeks (c.f. https://journals.lww.com/jcrjournal/fulltext/2023/11000/characteristics_and_treatment_of_exercise.4.aspx).
