Worked at "Big Blue" for over a decade, saw many of these cases, more common than people realize.

My role involved reviewing finances for medium-sized customers.

My hourly rate was ~$20 (quite good back then), but on the invoice sent to the customer was ~$8500.

That is ~425x.
