Apparently #barotrauma is actually open-source, you just buy the compiled versions. So I compiled it for a friend who can't afford the $50 steam version (and I wouldn't either - I didn't buy it for myself, someone gave it to me as a steam gift)

That's also more or less what you do with ZDoom and Chocolate Doom and other forks of iD's original source: you run them using the original levels and art and whatnot from your bought copy. (Or you use a package of original, 3rd party, free-to-use levels and art and whatnot)

@mightyspaceman the original art? Well, yeah. It's functionally identical to just copying the game in the first place.
